This is my little idea about gear 4th power up(if there will be one).
the basic of gear 4th is density. It means, on this stats Luffy's rubber mass get extremely denser than normal. It's so dense that Luffy's body looks overflowing(Imagine Venom or Carnage from Spiderman, something like that)
His speed is a bit slower than normal but faster than gear 3rd...
And his power approaching gear 3rd...
general sword's slash can't hurt him...
the speciality of this state, He could even use his forehead skin to stretch and attack foe with high damage, It's all because of his dense rubber mass.
The attack's names, like gear 2nd and 3rd, is accesoried with word 'Akuma' (means devil). Like "Akuma Pistol"
I have pictured one of his attacks, maybe I'll submit it later...

ACTIVATION : Luffy need to learn Seimei Kikan/Life return ability or anysimilar technique, and use it to activate gear 4th

Now... emmm... where to start... It looks fantastic! The colouring... this yellow, dark and red combination is very dangerouslooking and very powerful! The shadows are very ominous! Swords don't work and forehead skin?!

This is the problem of all the other Gear 4 theories, they are al ltoo lame. Oda wouldn't make something lame, it would be something totally new and awesome like this. I already thought what is the minus. The whole life then.
